Welcome new families! Our office is paperless and digital and we want to make completing our forms and understanding our policies an easy and comfortable process for everyone. We invite you to do this in the comfort of your home. On the day of the visit, we will review the forms together and ask you to e-sign the documents on our signature pad.
Follow these 1-2-3 steps and it will be smooth sailing from here on!
Step 1: Complete the online registration form for each of your children. We kindly ask that you do this at least 48 hours prior to the appointment so that our team can review the information.

At Smiles Pediatric Dentistry, we make every attempt to schedule appointments at your convenience. Allotted Appointment times are designed to fit your child’s specific needs. Infants, toddlers and special needs children are usually scheduled in the morning when they are alert and more cooperative. Young children with extensive dental needs are also preferably seen in the morning. This allows parents more time during the day to make sure their child is comfortable after the session. Parents also have the chance to call us with any questions or concerns the same day of treatment. The California State Education Code (Article 2, Section 46010(b)3) designates medical, dental and vision appointments as excused absences. Students missing only part of the school day are to be counted as in attendance. We will be happy to provide schools with letters explaining the necessary dental treatment.
Since appointed times are reserved exclusively for each child, we ask that you please notify our office 48 hours in advance of your scheduled appointment time if you are unable to come in. Another child who needs our care could be scheduled if we have sufficient time to notify their family. We realize that unexpected things can happen, but we ask for your assistance in this regard.
We practice in an open family setting and believe that the dental appointment should be a pleasant experience for parents and children together. We invite you to be part of your child’s dental examination so that we can share our dental findings with you. We believe in demonstrating oral hygiene procedures to ensure you are able to help your child complete them comfortably at home and to motivate the entire family to focus on oral health. To better understand how we care for your child, parents are also involved in dental treatment.
Each child at Smiles Pediatric Dentistry is unique. A carefully tailored plan will be created for your child after a comprehensive examination is completed, individualized plans address specific dental needs and include prevention, hygiene instructions, diet counseling in addition to treatment of dental problems.
Our administrative team will be assisting you to clearly understand our office policies. We believe in transparency, ease and straightforward contracts.
We will be explaining what fees to expect prior to your visit. Payments are due on the day of treatment. If you wish to utilize dental insurance benefits, we will require the insurance information at least 48 hours in advance of the visit. This time frame allows our team to research your plan benefits as a courtesy and to provide you with a general estimate of coverage.
Please keep in mind that your insurance contract is between your insurance company and you or your employer. We are considered a third party and, at times, the smoothest route is for you to pay our office directly and receive a personal reimbursement check from your carrier.
